I am seeking a CORU registered Social Worker to work with older persons in the community.

Contract Position
Main Purpose of role:
To provide a social work service for service users, working as part of a multi-disciplinary team, to support and maximise their quality of life and to ensure agreed multidisciplinary care plans are implemented. To work proactively with other agencies to ensure the best outcomes for service users.
Requirements:
- Social Work degree
- CORU Registration
- Own transport and full licence
- Knowledge and experience of Assisted Decision Making (Capacity) Act, including DSS, DMRR process
- Knowledge and experience of Nursing Home Support Scheme (Fair Deal)
