Patient Flow Coordinator (CNM2 Grade) (monday to friday role)

Are you an experienced registered nurse with a strong background in acute care, patient assessment, and bed management? This is an exciting opportunity to join a brand-new, state-of-the-art healthcare facility in Limerick and play a key leadership role in coordinating admissions, discharges, and patient flow.
You’ll work closely with multidisciplinary teams, community services, and senior clinical leadership to ensure seamless transitions of care and optimal bed capacity management.
About the Role
As Patient Flow Coordinator (CNM Grade), you will be responsible for:
- Managing referrals, admissions, discharges, and bed occupancy
- Conducting clinical assessments of patient referrals
- Coordinating discharge planning with MDTs and community services
- Producing data reports and analysing bed utilisation trends
- Building strong relationships with hospitals, home support services, and external agencies
- Supporting strategic planning and efficient use of capacity
- Contributing to governance, risk management, infection prevention, and quality improvement initiatives
This role combines clinical expertise, operational coordination, and leadership.
What You’ll Need
Essential Requirements
- Current registration with the Nursing and Midwifery Board of Ireland (NMBI)
- Minimum 5 years’ post-registration nursing experience
- At least 2 years’ experience in acute or related specialist care
- Strong understanding of patient flow, bed management, and discharge planning
- Excellent clinical assessment and decision-making skills
- Proven administrative and data analysis abilities
- Eligibility to work in Ireland
Desirable Qualifications
- Management qualification
- Teaching and Assessing qualification
- Post-registration qualification in a relevant area
- Strong IT and networking skills
- Knowledge of the ageing process and community care pathways
