Role Summary
The post holder provides clinical leadership to support safe, timely patient discharge and effective hospital flow. They oversee discharge planning from admission to discharge, manage the patient lounge, and work closely with the multidisciplinary team to ensure coordinated, high-quality care.
Key Responsibilities
- Assess patients and plan safe discharge to appropriate care settings
- Liaise daily with ward teams to identify discharges and set plans
- Coordinate and lead multidisciplinary and complex discharge meetings
- Work with patients, families, and community services to support discharge
- Monitor length of stay and improve patient flow processes
- Oversee patient lounge operations and staff
- Ensure accurate documentation and smooth patient transitions
- Support staff development and promote best practice
- Ensure compliance with policies, standards, and quality initiatives
Requirements
- Bachelor’s degree in Nursing
- Registered with the Nursing and Midwifery Board of Ireland
- Minimum 5 years acute experience, including 2 years in management
