Job Overview
The Role
My client, a global healthcare company is urgently seeking a Homecare Co-ordinator to join their team based in Balbriggan, North County Dublin on a 9-month fixed term contract, with the possibility of being made permanent. This is a full-time, office based, position.
As a Homecare Co-ordinator you will be responsible for coordinating and assisting with the management of patients on the organisations homecare service. You will report directly to the Homecare Manager.
Tasks & Duties
- To gain a thorough working knowledge of the organisations Homecare service and patient management process.
- To manage and resolve all home patient/carer queries daily.
- To place weekly Home Parenteral Nutrition orders.
- To order ancillary items for patients and always ensure adequate stock levels.
- To make courtesy care calls to patients/carers.
- Responsible for the supervision, maintenance and filing of relevant documentation and for the accurate maintenance of patient records.
- Liaison with healthcare professionals on aspects related to patient care i.e., funding, cessation of feeding etc.
- To manage and coordinate patient training requirements with the Nursing team.
- To monitor and request new/renewed prescriptions and liaise with the relevant Dietitian/Pharmacist if prescription has not been received.
- Stability statements
- Organise all funding and required purchase order (PO) numbers for home enteral and parenteral patients.
- Monthly invoicing for HPN patients in ROI and Northern Ireland.
- To arrange holiday/export orders, as required.
- To provide statistics on patient numbers, as required.
- Responsible for bringing to the attention of the Manager any unusual situations complaints or patient safety concerns.
- Manage PN pump supply & servicing to ensure a fully accurate pump database is maintained and that there is 100% traceability always on all home feeding pumps.
- Managing external stakeholders.
- Any other duties which may be reasonably required.
Experience, Knowledge & Qualifications
- At least 3 years' experience in a customer service/office environment.
- Excellent MS Office skills (Excel, Word, Powerpoint & Databases)
- Excellent communication skills, both written and verbal.
- Demonstrated experience in service delivery, service development, project management and multi-tasking.
- Very strong interpersonal skills.
- Administrative experience in a healthcare environment is desirable.
- Pharmacy technician experience is ideal but not essential.
- Previous experience in dealing with patients in a busy customer care role.
