Location: Co. Galway - West Galway
Type of role: permanent role.
Working hours: 37.5h/wk (full time), Monday - Friday: 8.30am-5pm (core hours)


Job summary:
An exciting opportunity to work as part of the ICPOP in West County Galway an experienced Clinical Nurse Specialist.
The aim of the integrated Care Programme for Older Persons is to develop and implement integrated services and pathways for older people with complex health and social care needs, shifting the delivery of care away from acute hospitals towards community based, planned and coordinated care.
The objective of the programme is to improve the quality of life for older people by providing access to integrated care and support that is planned around their needs and choices, supporting them to live well in their own homes and communities


Purpose of the role:
Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008: * Clinical Focus * Patient/Client Advocate * Education and Training * Audit and Research * Consultant.


The successful candidate will work as a key member of the multidisciplinary ICPOP team providing a patient centred quality service and this specialist practice will encompass a major clinical focus which comprises assessment, planning, delivery and evaluation of care given to patients and their families.


Your profile:

  • Third level (BSc) Nursing qualification
  • You will also have completed a Post grad qualification in Elderly care ideally, however some other post grad qualifications might be considered
  • You will possess an active registration with the Nursing and Midwifery Board of Ireland Register NMBI.
  • You will have completed a post registration programme of study, as certified by the education provider, which verifies that the applicant has
  • achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 8 or higher award or can provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 8 or higher standard, relevant to the specialist area of care
  • You will have a minimum of 5 years post registration full time experience post qualification
  • You will have a minimum of 2 years' experience in Care of the elderly.
  • You will have access to your own vehicle as travel is part of this role
  • You will possess a full driving licence
  • You will speak fluent English and be eligible to work in Ireland.
