Job Overview

Basic Grade Pharmacist required for a hospital in Waterford for a full-time permanent position

Provide effective, efficient, and safe pharmaceutical care to all patients under their care in the hospital under the direction of the chief pharmacist. Identify and address individual patient care needs where appropriate. Advise and provide information to other healthcare professionals, including other pharmacists, to assist them in their care of the patient. Any other duties assigned to him or her by the chief pharmacist.

Duties and Responsibilities

  • Provide a clinical pharmacy service; be an active member of the multidisciplinary team; provide drug information to medical, nursing and healthcare professionals, and patients; carry out prescription review, patient education, and documentation of clinical interventions.
  • Participate in the development and expansion of clinical pharmacy services.
  • Ensure best practice and compliance with agreed guidelines for the use of medications, developing and/or revising these where assigned by the chief pharmacist.
  • Participate in and develop the antimicrobial stewardship programme.
  • Participate in the medication safety programme, including the monitoring, reporting, and investigation of adverse drug reactions, medication incidents, and errors.
  • Work collaboratively with other members of the team to ensure that patient care is central to all activities of the Department of Pharmacy.
  • Participate in service delivery as part of a professional, punctual, and dedicated team.
  • Create, promote, and maintain open communications and healthy working relationships.
  • Ensure the best pharmaceutical practice, having regard for legal and ethical requirements for the provision of the pharmacy service.
  • Maintain the principles of equity, accountability, quality, and safety in daily work.
  • Work collaboratively with other pharmacy department staff to deliver and develop clinical pharmacy services to achieve stated goals and objectives, respecting the role of other pharmacists in ensuring safe and effective care.
  • Manage and develop self and others in a busy working environment.
  • Have the skills necessary to evaluate information, make effective decisions, and solve problems.
  • Have the ability to manage time, deadlines, and prioritise the workload.
  • Assist in managing the efficient running of the dispensary.
  • Participate in and supervise the operation of the dispensary and the procurement, storage, and supply of pharmaceuticals and other related items.
  • Ensure accurate records relating to the procurement, storage, compounding, quality control, dispensing, distribution, and destruction of medical products are maintained in accordance with best-practice standards and applicable legislation.
Health and Safety
  • Ensure that work is carried out in a safe manner in accordance with the provisions of the Health, Safety and Welfare at Work Act and/or other related legislation.
  • All staff are responsible for identifying, assessing, and reporting all risks and for contributing to the management and review of all risks.
  • In cooperation with pharmacy staff and health care professionals work to reduce the risk of medication errors and adverse incidents, and other clinical risks associated with pharmacy.
Professional and Education
  • Maintain, enhance, and develop knowledge, skill, and aptitudes necessary to respond to changing clinical profiles of patients, hospital, and service developments and legal requirements, such as licensing.
  • Be willing to learn. Actively participate in professional development through self-learning, formal courses, and formal and/or informal on-the-job training.
  • Undertake continuing education and training to maintain a high level of competency in clinical pharmacy. Fulfil the CPE requirements of the pharmacy regulator, the Pharmaceutical Society of Ireland and the Irish Institute of Pharmacy (IIOP).
  • Deliver training and education to medical, nursing, and pharmacy staff through induction days, study days, journal clubs, and other educational events.
  • Supervise and tutor pharmacy interns, pharmaceutical technician students, student pharmacists, and other students during work experience placements.
  • Participate in the operation of a computer-integrated management system for the pharmacy. Have good computer skills, including Microsoft Word, Excel, and PowerPoint.
  • Participate in stock-taking, invoice-processing, and other administrative procedures as assigned by the chief pharmacist.
  • Ensure, in co-operation with the chief pharmacist and other staff, appropriate compliance with international and national guidelines, Pharmaceutical Society of Ireland, and other standards for the provision of pharmacy services and actively participate in internal and external quality control, quality assurance, audit, and accreditation.
  • Participate in the measurement and analysis of pharmacy activity.
  • Participate with the chief pharmacist in identifying and implementing operational processes to the standards of 'best practice' in order to optimise the use of resources and improve quality and medication safety.
  • Develop and implement policies, procedures, and guidelines relating to medicines management.
  • Be a registered Pharmacist with the Pharmaceutical Society of Ireland (PSI).
