We are recruiting on behalf of a leading specialist service in Galway supporting individuals with Acquired Brain Injury (ABI). This is a permanent, part-time (30 hours per week) opportunity for a CORU-registered Social Worker to join a highly skilled multidisciplinary team (MDT).
You will play a key role in supporting clients through their rehabilitation journey, with a strong focus on discharge planning, housing applications, and community reintegration.
Key Responsibilities:

Collaborate with the MDT on individualised discharge plans

Assist clients and families with housing applications and related supports

Advocate for client needs within community and statutory services

Provide emotional and practical support during transitions
Requirements:

CORU registration as a Social Worker (essential)

Experience in ABI, disability, or hospital/community social work (desirable)

Strong communication, advocacy, and team collaboration skills

Commitment to person-centred care and rehabilitation
