Overview
Join a specialist Community Intervention Team delivering rapid-response physiotherapy as part of a blended allied health service. This role supports adults in early discharge from hospital or avoiding unnecessary admission, with care provided in primary care settings and patients’ homes
Key Responsibilities

Deliver physiotherapy in patients’ homes or local care centres

Support patients and carers to maximise independence in daily living

Work within a multidisciplinary team across acute, community, and primary care settings

Organise and prioritise caseload, liaise with referring clinicians
Essential Requirements

CORU registration as a Physiotherapist

Minimum 3 years post-registration experience

Full clean manual driving licence (vehicle provided)

Strong communication, decision-making, and IT skills

Proven team-working ability in multidisciplinary settings
Desirable Attributes

Experience in acute or primary care discharge support

Ability to manage caseloads independently and flexibly

Skilled in building rapport with patients and families
