Head of Quality, Patient Safety and Risk - Hospital – Dublin

Overall Objective:
The Head of Quality, Patient Safety and Risk plays a pivotal role in leading the hospital’s strategic and operational approach to quality, safety, and risk management. As a member of the Executive Management Team, this senior leadership role is responsible for ensuring a culture of continuous quality improvement and robust risk governance across all adult and paediatric services.
Key Responsibilities Include:

Strategic Leadership: Shape and implement the hospital’s vision for quality, patient safety, and risk, ensuring alignment with national standards (e.g., HIQA, HSE Patient Safety Strategy) and the hospital’s strategic goals.

Clinical Governance & Risk Management: Lead clinical risk systems and structures, including oversight of risk registers, incident reporting, root cause analyses, and mitigation strategies. Provide regular reports and recommendations to senior leadership and external bodies.

Quality Improvement: Drive hospital-wide quality initiatives, audit processes, and service evaluations. Use data trends, patient feedback, and performance indicators to identify improvement opportunities and lead implementation.

Patient Safety & Compliance: Monitor adherence to national regulatory frameworks, manage serious incident reviews, and ensure timely reporting to relevant stakeholders including the State Claims Agency and Coroner's office.

Leadership & Team Development: Provide support and guidance to the Quality and Risk teams, promoting a culture of learning, open disclosure, and continuous professional development.

Governance Support: Support clinical governance committees, including the Senior Incident Management Forum (SIMF), by ensuring the timely provision of data, insights, and actionable recommendations.

Training & Education: Oversee training in quality and risk practices, open disclosure, and incident management frameworks across the hospital.

Stakeholder Engagement: Act as a key liaison with internal stakeholders, regulatory bodies, external agencies, and national quality programmes, representing the hospital as required.

Corporate Responsibilities: Contribute to recruitment, contract management, sustainability efforts, and ensure policy compliance throughout the organisation.
This role is central to embedding a high-performance, safety-first culture that promotes excellence in patient care and regulatory compliance across all hospital functions.
Qualifications, Experience & Person Specification
Each candidate must by the latest date of application process:
- A minimum of 5 years Senior Managerial Experience working in an acute Hospital in-patient setting, in a post that has involved quality and patient safety management as relevant to this role
And
- A 3rd level qualification in a clinical field
Or
- Hold a QQI academic award at Level 8 or higher in Risk Management or Quality in Healthcare on the Quality and Qualifications Ireland (QQI) framework or equivalent.
And
- Have experience of Incident Management processes and subsequent System Analysis Investigation processes.
- Experience of leading and implementing Quality improvement initiatives within a Health setting.
- Experience of changing organisational culture regarding Quality and Patient Safety through the delivery of education and training.
- Experience of Team Leadership and collaborative working with multiple stakeholders.
