Care Co-ordinator – Older Persons Services
FRS are recruiting on behalf of our client for an experienced Care Co-ordinator to support the delivery of high-quality home care services.
This role involves managing care rotas, allocating care staff efficiently, and ensuring customers receive safe, consistent, and person-centred care in their own homes. You will respond to schedule changes, arrange cover when required, and communicate effectively with both customers and the care team.
Key responsibilities include:

Coordinating and allocating care staff in line with care plans

Managing rotas and arranging cover for sickness and holidays

Processing new referrals and schedule changes

Using care management systems to match staff skills to customer needs

Communicating changes clearly with customers and staff

Maintaining accurate records and confidentiality

Participating in an on-call rota to ensure continuity of care
